PA for a female person with early onset dementia in SY2

I am a a 53 year old woman who used to work for the NHS as a physiotherapy assistant. I grew up in East Sussex and moved to Shropshire when my rare dementia began to affect my wellbeing.
I am living independently in my own flat near to Shrewsbury Town centre and enjoy walking into town daily and visiting shops. I love Swimming, listening to music and watching DVD's.
I now need a little help with maintaining my personal hygiene and my clothing. My dementia affects my ability to plan and organise my routines. My memory is not my main difficulty. However processing information is now becoming more difficult. Although I am able to understand what I should do, my brain does not prompt me to carry out tasks effectively. I now need a few hours a week to help me to bathe, help with laundry and other small tasks. As my condition progresses I will need additional help.
I have family who are involved in my life and when the weather is clement I enjoy going for a walk at Attingham.
My preference is for a self employed PA. the hourly rate of £19 for a self employed PA will be payable. Initially I will need 4 hours a week whilst I become used to receiving care. Days and hours can be flexible to suit both parties for the right candidate.
